Defect Elimination (DE) is a structured process for finding the root cause of recurring failures and removing it permanently. The defining word is "permanently" — a DE outcome that does not survive the next 90 days of operation has not been completed.
When failures are prioritized, analyzed by a cross-functional team, and solutions are implemented rigorously, the number of failures can be significantly reduced, and asset performance and reliability can be improved. The compounding effect of this is significant: fewer failures mean more production, lower cost, less unplanned downtime, and a safer plant.
In an immature organization, DE is a firefighting tool — reactive and crisis-driven. In a mature organization, DE is a precision instrument for continuous improvement. This guide gives you both the starting point and the destination.
